Tracking Patient PPS Measurements

This article explains how you can use Sumac Case Management for Hospices to keep track of PPS Measurements for patients.

During a patient's hospice stay, you may wish to record and track their PPS progress.  To record a PPS measurement, open the patient's Contact record and go to their History tab. Click "Add to Contact" and select "PPS Measurement."

  1. Select the Case Manager for the Patient. In most cases, this is the lead health care practitioner managing the patient's care program. 
  2. Ensure the correct patient Contact record is selected. 
  3. Record the date of the PPS measurement and in the PPS score dropdown, select the appropriate score. 
  4. In the Notes section, record any additional information and click OK to save the measurement.


For each new PPS measurement, a new record should be created in order to track their progress. If you edit an existing PPS record, you lose that history.

If you need to record a new PPS Measurement for multiple patients, you can also enter new PPS Measurement records directly in the PPS Measurement list. 
From the main Sumac console, expand Specialized, and click "PPS Measurements" to open that list. Click "New" to enter a new one. 
This will save you time when entering new PPS Measurement records for several contacts, as you won't have to take time to find each contact first!
